Go to FAA Medexpress website  medxpress.faa.gov and complete the application (FAA FORM 8500-8); 

1. Review and connect to MedXPress at https://www.faa.gov/pilots/safety/pilotsafetybrochures/media/medxpress.pdf
2. Create an account or login using your existing account 
3. Enter medical application data 
4. Submit application 
5. Print summary sheet 
6. Bring the summary sheet with you and present it along with your confirmation number at the front desk when you arrive at the office.
